The CCRA Adult Recreational League is intended for participants 18 years of age and older.

Participants under the age of 15 are not eligible to register using this form. Children ages 7–15 who wish to participate must complete the CCRA Youth Softball Clinic Registration Form found here:

Participants who are 16 to 17 years and 11 months of age may participate in the adult recreational division only with prior approval from CCRA and must have a parent or legal guardian complete all required parental acknowledgements and waivers, and/or a parent, legal guardian, or designated adult responsible for the minor child is also registered and participating on the same team.

All participants under the age of 18 must:

Have a parent or legal guardian review the registration and waiver documents.
Complete the Parent/Guardian Acknowledgement section located at the bottom of this registration form.
Have all required parental consents completed prior to participation.
Comply with all CCRA youth participation and safety policies.

CCRA reserves the right to request proof of age and to refuse or cancel registration if age eligibility requirements are not met.

I understand and acknowledge that the participant is registering to participate in an adult recreational sports league and may be participating alongside, competing against, interacting with, traveling with, communicating with, and attending activities involving adult participants who are 18 years of age or older.

I understand that participation in an adult recreational league may expose my child to risks and circumstances that are different from those associated with youth-only programming, including but not limited to:

• Physical contact and collisions with adult participants who may be larger, stronger, faster, or more experienced;
• Injuries resulting from gameplay, practices, drills, training sessions, tournaments, or recreational activities;
• Accidental contact involving bats, balls, equipment, or other participants;
• Differences in physical maturity, athletic ability, and skill level between participants;
• Exposure to adult conversations, behaviors, language, opinions, viewpoints, or conduct that may not always align with youth-oriented environments;
• Transportation, arrival, departure, and event-related risks;
• Injuries resulting from weather conditions, field conditions, facilities, equipment, spectators, or other participants;
• Risks associated with interactions among participants, coaches, volunteers, spectators, officials, and members of the public;
• Any other known or unknown risks reasonably associated with participation in adult recreational sports and community activities.

I acknowledge that no representation or guarantee has been made by CCRA regarding the prevention of injuries, accidents, incidents, misunderstandings, disputes, inappropriate conduct by third parties, or other occurrences that may arise through participation in an adult recreational environment.

I expressly consent to my child participating in the adult division and acknowledge that I have determined that such participation is appropriate for my child.

I further acknowledge and agree that a parent, legal guardian, or designated responsible adult approved by CCRA must be registered and participating on the same team as the participant at all times while the participant is eligible to participate in the adult division.

Assumption of Risk

On behalf of myself and the participant, I knowingly and voluntarily assume all risks, dangers, and hazards associated with participation in CCRA programs and activities, whether foreseen or unforeseen, including risks arising from the actions, omissions, negligence, conduct, or participation of other players, coaches, volunteers, spectators, officials, or third parties.

Release and Waiver

To the fullest extent permitted by applicable law, I hereby release, waive, discharge, and hold harmless the Canadian Christian Recreational Association (CCRA), its affiliated churches, directors, officers, employees, volunteers, coaches, instructors, chaplains, officials, umpires, team managers, league organizers, sponsors, partners, agents, representatives, members, participants, and property owners (collectively referred to as the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, liabilities, costs, expenses, or legal proceedings arising directly or indirectly from the participant’s involvement in CCRA activities.

This release includes, without limitation, claims arising from bodily injury, illness, emotional distress, property damage, disability, loss, death, participant interactions, spectator interactions, transportation-related incidents, and any other risks associated with participation, except where prohibited by law.

Indemnification

I agree to indemnify and hold harmless the Released Parties from and against any claims, demands, damages, liabilities, costs, expenses, or legal fees arising out of or relating to the participant’s actions, conduct, participation, or presence at any CCRA activity.

I acknowledge that participation in recreational sports and activities organized by the Canadian Christian Recreational Association (CCRA), including but not limited to softball and other athletic programs, involves inherent risks of injury, including but not limited to bodily injury, illness, permanent disability, or death, as well as potential damage to personal property.

In consideration of being permitted to participate, I hereby voluntarily assume all risks associated with my participation and agree to release, waive, and hold harmless the Canadian Christian Recreational Association (CCRA), its organizers, affiliated churches, team managers, volunteers, directors, officers, employees, coaches, umpires, assignes, agents, representatives, and sponsors (collectively referred to as the “Released Parties”) from any and all claims, demands, actions, damages, losses, or liabilities of any kind, whether arising from negligence or otherwise.

This waiver applies to any and all activities, events, travel, and participation associated with CCRA programs, including decisions made by the Released Parties in the course of organizing and administering such activities.

I acknowledge that failure to comply with this Code of Conduct may result in disciplinary action, including removal from games, suspension, or expulsion from the league without refund.

As part of normal CCRA programs, leagues, events, tournaments, activities, workshops, and recreational experiences, photographs and video recordings may be taken for promotional, educational, historical, and communication purposes.

Participants should be aware that images and video footage captured during CCRA activities may be used in CCRA publications, promotional materials, newsletters, social media platforms, websites, annual reports, presentations, and other organizational communications.

Photographs are typically taken in group settings, during recreational activities, game play, special events, award presentations, community gatherings, and other program-related experiences.

As a matter of practice, CCRA does not identify participants by name in photographs or videos unless separate permission has been obtained or the individual is participating in a specific featured story, interview, testimonial, or recognition initiative.

By participating in CCRA programs, leagues, events, and activities, participants acknowledge that photography and videography may occur as part of the overall recreational experience and that such images may be used by CCRA for organizational and promotional purposes.

CCRA is committed to using all photographs and media respectfully, appropriately, and in a manner consistent with its Christian values and mission.

Before registering, please take a moment to review the principles, values, and mission of the Canadian Christian Recreational Association (CCRA) Youth Softball Clinic.

This clinic is a faith-based Christian program rooted in biblical principles, Christian fellowship, prayer, mentorship, encouragement, and Christ-centered values. While this program is free of charge and absolutely open and welcoming to children and families of all backgrounds, beliefs, cultures, and experiences, participation in the faith-based components of the program is mandatory and forms an essential part of the clinic experience.

• Christian prayer before and/or during sessions
• Faith-centered discussions and encouragement
• Short devotionals or mini-sermons
• Bible-based principles

LIABILITY WAIVER AND RELEASE

On behalf of my child, I acknowledge that participation in recreational sports and activities organized by the Canadian Christian Recreational Association (CCRA), including but not limited to softball and other athletic programs, involves inherent risks of injury, including but not limited to bodily injury, illness, permanent disability, or death, as well as potential damage to personal property.

In consideration of being permitted to participate, on behalf of my child, I hereby voluntarily assume all risks associated with my participation and agree to release, waive, and hold harmless the Canadian Christian Recreational Association (CCRA), its organizers, affiliated churches, team managers, volunteers, directors, officers, employees, coaches, umpires, assignes, agents, representatives, and sponsors (collectively referred to as the “Released Parties”) from any and all claims, demands, actions, damages, losses, or liabilities of any kind, whether arising from negligence or otherwise.

This waiver applies to any and all activities, events, travel, and participation associated with CCRA programs, including decisions made by the Released Parties in the course of organizing and administering such activities.

Demonstrate respect toward players, coaches, volunteers, officials, and other families.

Refrain from abusive, disruptive, threatening, or inappropriate conduct.
Support CCRA’s Christian values of respect, encouragement, integrity, and community.
Follow all program rules, safety procedures, and volunteer directions.

Failure to comply with the Parent Code of Conduct may result in removal from the activity or future participation restrictions.

PARENT SUPERVISION REQUIREMENT: *

I understand and agree that a parent, guardian, or designated responsible adult must remain on-site and on the premises for the duration of the program.

IMPORTANT SUPERVISION REQUIREMENT

CCRA’s FREE Kids Softball Clinic and Games Program is designed as a family-supported recreational program.

Parents, guardians, or a designated responsible adult (18 years of age or older) are required to remain on-site and on the premises at all times during program hours.

This program is not a childcare service, daycare, or drop-off program. If a parent, guardian, or designated responsible adult leaves the premises, the child may not be permitted to participate.

On behalf of my child, , I acknowledge that participation in sports and recreational activities carries a risk of concussion and other head injuries, and I understand the importance of recognizing and properly responding to such injuries.

By completing my registration, I confirm that I have reviewed concussion education resources consistent with those provided by the Province of Ontario, and that I understand the signs, symptoms, and risks associated with concussions.

On behalf of my child, I further acknowledge that continuing to participate in sports after a concussion or suspected concussion may result in serious health consequences. I agree to immediately remove my child from participation and seek appropriate medical attention if my child experiences or there is suspicion of a concussion or head injury.

On behalf of my child, I agree to follow all concussion protocols, guidelines, and return-to-play requirements established by the Canadian Christian Recreational Association (CCRA).

PHOTOGRAPHY & MEDIA NOTICE

As part of normal CCRA programs, leagues, events, tournaments, activities, workshops, and recreational experiences, photographs and video recordings may be taken for promotional, educational, historical, and communication purposes.

Participants should be aware that images and video footage captured during CCRA activities may be used in CCRA publications, promotional materials, newsletters, social media platforms, websites, annual reports, presentations, and other organizational communications.

Photographs are typically taken in group settings, during recreational activities, game play, special events, award presentations, community gatherings, and other program-related experiences.

As a matter of practice, CCRA does not identify participants by name in photographs or videos unless separate permission has been obtained or the individual is participating in a specific featured story, interview, testimonial, or recognition initiative.

By participating in CCRA programs, leagues, events, and activities, participants acknowledge that photography and videography may occur as part of the overall recreational experience and that such images may be used by CCRA for organizational and promotional purposes.

CCRA is committed to using all photographs and media respectfully, appropriately, and in a manner consistent with its Christian values and mission.
