LIABILITY WAIVER AND RELEASE



I acknowledge that participation in recreational sports and activities organized by the Canadian Christian Recreational Association (CCRA), including but not limited to softball and other athletic programs, involves inherent risks of injury, including but not limited to bodily injury, illness, permanent disability, or death, as well as potential damage to personal property.



In consideration of being permitted to participate, I hereby voluntarily assume all risks associated with my participation and agree to release, waive, and hold harmless the Canadian Christian Recreational Association (CCRA), its organizers, affiliated churches, team managers, volunteers, directors, officers, employees, coaches, umpires, assignes, agents, representatives, and sponsors (collectively referred to as the “Released Parties”) from any and all claims, demands, actions, damages, losses, or liabilities of any kind, whether arising from negligence or otherwise.



This waiver applies to any and all activities, events, travel, and participation associated with CCRA programs, including decisions made by the Released Parties in the course of organizing and administering such activities.

CODE OF CONDUCT AGREEMENT



I agree to conduct myself in a respectful, sportsmanlike, and Christ-centered manner at all times while participating in any Canadian Christian Recreational Association (CCRA) activity.



I understand that I am expected to:


Treat all participants, officials, volunteers, and spectators with respect and dignity
Demonstrate good sportsmanship and fair play
Refrain from any form of harassment, discrimination, or inappropriate conduct
Follow all rules, guidelines, and decisions made by league officials and organizers



I acknowledge that failure to comply with this Code of Conduct may result in disciplinary action, including removal from games, suspension, or expulsion from the league without refund.

I acknowledge that participation in sports and recreational activities carries a risk of concussion and other head injuries, and I understand the importance of recognizing and properly responding to such injuries.



By completing my registration, I confirm that I have reviewed concussion education resources consistent with those provided by the Province of Ontario, and that I understand the signs, symptoms, and risks associated with concussions.



I further acknowledge that continuing to participate in sports after a concussion or suspected concussion may result in serious health consequences. I agree to immediately remove myself from participation and seek appropriate medical attention if I experience or suspect a concussion or head injury.



I agree to follow all concussion protocols, guidelines, and return-to-play requirements established by the Canadian Christian Recreational Association (CCRA).

I acknowledge that the Canadian Christian Recreational Association (CCRA) will collect, use, and store my personal information for the purposes of registration, program administration, communication, safety, and organizational operations.



I consent to the collection, use, and disclosure of my personal information in accordance with CCRA’s Privacy Policy, including communication related to league updates, schedules, events, and organizational activities.



I understand that reasonable measures will be taken to protect my personal information and that it will not be shared beyond what is necessary for program delivery, legal compliance, or organizational operations.



I further consent to the use of photographs, videos, or media content captured during participation for promotional, marketing, and community engagement purposes, unless I provide written notice to opt out.

By completing this registration, I confirm that I am voluntarily participating in programs offered by the Canadian Christian Recreational Association (CCRA) and that I am physically able to participate.



I agree to abide by all rules, policies, procedures, and by-laws established by CCRA, including those related to safety, conduct, and participation.



I acknowledge that registration fees are subject to the organization’s no refund policy as outlined on the registration platform, program credits only.



I further understand that CCRA reserves the right to make decisions regarding scheduling, league structure, and program delivery in the best interest of participants and the organization.
